Tribute License Plates Application Instructions

Thank you so much for your decision to purchase  “IN REMEMBRANCE, APRIL 16, 2007” Virginia license plates, to honor the victims and survivors of the tragedy, and for your help in making university and college campuses safe and more secure! 

IMPORTANT NOTES:

  • Do not mail this application to the Virginia DMV. Mailing instructions provided later in this document
  • You must complete ALL four steps below to complete your order. 

Instructions:

1.   Click here then print a hard copy of the Virginia’s License Plate Application Form.  

2.  Fill out the State of Virginia License Plate Application Form in black or blue ink:

a.   Application Type Section: 

  • Verify “Vehicle License Plates” box is checked

b.   License Plate Type Requested Section:

  • Verify the box “Other” is checked and contains “In Remembrance, April 16, 2007”
  • If you seek personalized plates, check the box “Personalized License Plates”, review your printed Virginia “Personalized License Plates Policy”, then enter your personalized  license plate choices
c.   Applicant Information Section:
  • Fill in the information up through “Vehicle Title Number” and “Vehicle ID Number”
d. Certification/Signatures Section:
  • Check one of the two boxes that apply to your regarding liability insurance of the vehicle
  • Applicant (and Co-Applicant if applicable) sign/date the form

3. Application Delivery Instruction:

4. Payment Instructions:

Select your method of payment: (Non-Personalized Plates: $25; Personalized Plates: $35)
  • Personal Check:  Made check payable to “VTV Family Outreach Foundation”  with “April 16 License Plates” annotated in the memo line of your check
  • Pay Online:

  • Buy Non-Personalized Plate ($25)

  • Buy Personalized Plate ($35)
